The following modem chipsets would work with Asterisk:

 - Ambient MD3200
 - Motorola 62802
 
To give power over ethernet use the following schema
1 TX+ 2 TX- 3 RX+ 4 DC- 5 DC- 6 RX- 7 DC+ 8 DC+
Give pair 4 and 5 a negative current and give pair 7,8 a positive current. Usually the current is 48V
